According to police, the two vehicle collision occurred just after 3 p.m. on July 28 in the Municipality of Bluewater.
The wanted vehicle is a blue pick up that was towing a trailer at the time of the incident.
Anyone with information is asked to contact police or report anonymously to Crime Stoppers.
