The Ministry of Labour is investigating a fatal industrial accident at a tool and die shop in Oldcastle.
The ministry was notified on Monday that an employee of Saturn Tool & Die was injured by a piece of machinery and later succumbed to their injuries.
Few other details are available, but two ministry inspectors have been assigned to the investigation.
One order has already been issued, however the investigation is ongoing.
